computer mood tracker

Does anyone know of an online program or software that will let me keep track of the meds and herbs I'm taking as well as let me note my anxiety level, social life, depression, etc...? I'd like to see if my life is improving or at least is pretty stable over time. I don't think I recall the past very well.

I don't know any specific program but it can easily be done in eg. ms Excel.
