Breakthru Panic attacks - increase the klonopin?

Posted by Jackster on June 28, 2003, at 1:15:55

I been working my way thru my exposure hierarchy (I have agoraphobia) - and have just increased from 0.5mg of klonopin a day to 1mg as things started to get harder. I'm still having break through panic attacks doing the harder stuff on my hierarchy (although they're not as severe) - should I up the dose of klonopin again? Or maybe just continue with the 1 mg for a week or two more?

Thanks
Jackie

Hi Jackie, you might wait another week first, because Klonopin tends to accumulate quite a bit, so your blood level will continue to rise for a week or so after a dose increase, then reassess another increase then. The last spell of panic attacks I experienced (a couple of years ago), I had to take 1.5mg/day for a couple of weeks to get them under control.

Hi,

I also have anxiety with ocd. I'm on 225mg of effexor and I was on .5 clonazepam 1/2 in the morning and a whole in the night. This regime worked well for about 5 months and then I started to notice more and more anxiety creep in until I was having it all day again. My pdoc said I could take .5mg of clonazepam 3 times daily if I needed. This was 4 weeks ago and I increased it 1/2 little by little because I'm so nervous of taking all this medicine. Right now I'm taking 1/2 in morning and .5 in afternoon and .5 in evening, I'm feeling alitte relief but not like before. I guess I need reassurance about taking .5 three times a day. My fear is being a anxiety resistant person and having to be so dope up to get any relief.

>What about adding a low dose of xanax, prn when you have the breakthrough attacks-it works really quickly and has a really short half-life. Just a thought.

The Xanax idea is a good one -- that's what I do, and the effect is very quick, especially if you dissolve it under your tongue (although the pills are quite bitter).

I find that as little as 0.25-0.50 mg does the trick (I don't use it very often), and it doesn't seem to interact with the Klonopin. My pdoc is fine with this.
