Everbody's different of course, but i personally think that 40mg ritalin a day is a lot. Ask yourself if you're too nervous, moody, paranoid or mildly delusional at your current dose. If not, and it's helpful, maybe an increase might be good. i know i wouldnt like that much ritalin (40+ mg) in me on a daily basis though.

I think 40mg/day of methylphenidate is fairly average.
How long have you been at that dosage?
Also, my previous pdoc HATED Ritalin SR, he would not write for it. He told me I could have Ritalin IR, or Concerta, or Metadate, but not the SR. He said that it was basically a wax tablet and that it's dissolve rate and delivery of MPH was very erratic and unreliable.
So, if you can afford it, you may want to ask your pdoc for the 54mg Concerta tablet. That would be a slight dose increase, plus a top quality controlled release mechanism.

i have read 20 mg ritalin SR is equivalent to 7 mg regular ritalin ...
so i should ask -- is 7 mg twice a day a lot?
do psychotic symptoms come on gradually for a bipolar?

hence i am considering a move to the UK (where psychiatrists are not available) :)

I don't know where you get those numbers from; it doesn't sound right to me (20mgER equivalent to 7IR). 7mg. of ritalin IR twice a day is not a lot.

my pdoc had me taking 60 mg of ritalin daily - not sr though. i only can handle 20 mg in the morning now though due to increased sweating, a crazy sugar craving and 2-3 naps a day. im thinking about stopping altogether though because im starting to binge on high sugar snacks and have gained 30 lbs since introducting ritalin to my prozac treatment.
